4C Health is a Certified Community Behavioral Health Center (CCBHC) in Indiana, originally founded in 1975 as Four County Comprehensive Mental Health Center, serving as the designated community mental health provider for Cass, Miami, Fulton, and Pulaski counties, and expanding to surrounding rural areas including Howard, Tipton, White, Carroll, Clinton, Marshall, Wabash, and Grant counties. The organization offers a comprehensive continuum of integrated behavioral health and primary care services, including hybrid treatment options (in-person, virtual, and phone) for mental health and substance use recovery, 24/7 crisis intervention with walk-ins, mobile crisis teams, and immediate access for intravenous substance users or pregnant individuals; inpatient psychiatric unit and psychiatric urgent care in Peru; psychiatric evaluations and medication management; on-site pharmacy partnerships; outpatient therapy for all ages using person-centered approaches like CBT, EMDR, and play therapy, plus psychological testing; community-based skills training and wraparound services for families; school-based supports in over 20 corporations; addiction recovery with MAT, peer supports, and groups; adult intensive programs like Assertive Community Treatment and clubhouse models; residential options including supervised group living and transitional housing; supported employment; and prevention services such as suicide prevention training, substance use education, and HIV testing.
