4C Health is a Certified Community Behavioral Health Center (CCBHC) in Indiana, dedicated to supporting ALL Hoosier well-being through high quality, whole health care that is compassionate, collaborative, and competent.

Originally founded in 1975 as Four County Comprehensive Mental Health Center serving Cass, Miami, Fulton, and Pulaski counties, it has expanded since 2015 to include additional rural communities in North Central Indiana, such as Howard, Tipton, White, Carroll, Clinton, Marshall, Wabash, and Grant counties, leading to a rebranding in 2022 that reflects its roots in the '4' core counties and commitment to the '4Cs' values: CARE first for clients, colleagues, community, and self; COMPASSION toward clients and colleagues; COLLABORATIVE efforts to work hard and together; and COMPETENCE through ethical, data-supported, evidence-based interventions.

As one of eight designated CCBHCs in Indiana, 4C Health provides a comprehensive continuum of integrated behavioral health and primary care services for individuals of all ages, including crisis intervention with 24/7 support via phone at (800) 552-3106 or text 'IN' to 741-741, psychiatric urgent care, therapy (individual, couples, family), medication management, addiction recovery with Medication Assisted Treatment (MAT), community-based and school-based skills training, adult intensive services like Assertive Community Treatment, residential programs for independent living, supported employment, and prevention initiatives to address mental health and substance use disorders.

Embracing a hybrid care model that combines in-person, virtual, and phone options, 4C Health meets clients where they are—whether at home, school, or in the community—fostering long-term recovery, community safety, and whole-person health for underserved populations, including those struggling with intravenous substance use, pregnant individuals, and families in need.

Levels of Care

As one of Indiana’s designated Certified Community Behavioral Health Centers, 4C Health - Pulaski County delivers care through a highly adaptable hybrid model. The continuum spans from 24/7 Psychiatric Urgent Care and assertive community treatment to outpatient therapy and Supervised Group Living. Clients can engage in services in-person across numerous North Central Indiana micro-clinics, at home, or virtually via telehealth.

The multidisciplinary clinical team applies evidence-based modalities like EMDR, Cognitive Behavioral Therapy, and Motivational Interviewing. Care interventions are fully integrated. This means therapeutic modalities are frequently combined with psychiatric medication management and onsite pharmacy coordination to support whole-person health.

Addictions

Addiction recovery protocols prioritize immediate, life-saving intervention. They offer walk-in, same-day access for pregnant individuals and those struggling with intravenous substance use. Their specialized MaT-Rx program combines medication-assisted treatment with wraparound outpatient support to address opioid use and other dependencies.

Conditions

Clinicians here address a wide spectrum of mental health needs. They treat anxiety, depression, and trauma alongside severe and persistent mental illnesses. For individuals managing highly complex psychiatric conditions, the facility utilizes an Assertive Community Treatment model to reduce hospitalization risks.
