4C Health - Fulton County
4C Health, formerly known as Four County Comprehensive Mental Health Center since its founding in 1975, serves as the designated Community Mental Health Center for Cass, Miami, Fulton, and Pulaski counties, with expansion since 2015 to include Howard, Tipton, and White counties in North Central Indiana, and became a Certified Community Behavioral Health Center in 2024. Committed to supporting all Hoosier well-being through high quality, whole health care that is compassionate, collaborative, and competent, 4C Health embodies its core values of CARE, COMPASSION, COLLABORATION, and COMPETENCE to provide integrated behavioral health, substance use recovery, and primary care services to individuals, families, children, and adults across rural communities. The organization offers a wide array of services including 24/7 crisis intervention and walk-in access, hybrid therapy and psychiatric care via in-person, virtual, or phone options, school-based and community-based supports, addiction recovery with medication-assisted treatment, adult intensive programs like Assertive Community Treatment and Stepping Stones Clubhouse, residential living options, supported employment, street outreach for the unhoused, and prevention initiatives to address mental health stigma and substance use. With a person-centered approach utilizing evidence-based practices such as cognitive behavioral therapy and EMDR, 4C Health meets clients where they are—at home, school, or in the community—to promote long-term recovery, independence, and community safety, ensuring accessible care regardless of insurance status through financial assistance and partnerships.
