4C Health, formerly known as Four County Comprehensive Mental Health Center since its establishment in 1975, serves as the designated Community Mental Health Center for Cass, Miami, Fulton, and Pulaski counties, with growth since 2015 to include Howard, Tipton, and White counties, becoming a Certified Community Behavioral Health Center for these seven rural Indiana counties in 2024. Rebranded in 2022, 4C Health reflects its roots in four North Central Indiana counties while emphasizing whole person health through high quality, compassionate, collaborative, and competent care that supports all Hoosier well-being. Guided by core values of CARE—prioritizing clients, colleagues, community, and self with coordination and patient safety—COMPASSION by being present and providing hope, COLLABORATION through inclusive teamwork, and COMPETENCE via ethical, evidence-based practices, the organization offers a wide array of services including hybrid behavioral health treatment via in-person, video, or phone options; clinical services such as 24/7 crisis and walk-in support, psychiatric urgent care and stabilization, medication management, primary care integration, individual/family/group therapy using approaches like CBT and EMDR, community-based skills training, school-based mental health supports for over 20 school corporations, addiction recovery with MAT and peer support, adult intensive services like Assertive Community Treatment and clubhouse programs, residential options including supervised group living and transitional housing, supported employment, street outreach for the unhoused, and prevention initiatives addressing mental health stigma, suicide prevention, and substance use education. Targeting individuals across all ages facing mental health challenges, substance use disorders, co-occurring conditions, families, children in schools, the underserved rural populations, pregnant individuals, and those with intravenous substance use who receive immediate access, 4C Health ensures flexible, accessible care without compromising quality, partnering with pharmacies, law enforcement, schools, and communities to foster recovery, independence, and community safety.
