4C Health, formerly known as Four County Comprehensive Mental Health Center since 1975, is a designated Community Mental Health Center serving the rural communities of Cass, Miami, Fulton, Pulaski, Howard, Tipton, and White counties in North Central Indiana, with a mission to support all Hoosier well-being through high-quality, whole health care that is compassionate, collaborative, and competent. As a Certified Community Behavioral Health Center since 2024, 4C Health provides integrated behavioral health and primary care services to individuals of all ages facing mental health challenges, substance use disorders, and co-occurring conditions, emphasizing a person-centered approach that treats the whole patient. Their innovative hybrid care model offers flexible in-person, virtual, and phone options for therapy, psychiatric services, medication management, and crisis intervention, including 24/7 walk-in access, mobile crisis teams, and a Psychiatric Urgent Care/Crisis Stabilization Unit in Peru, Indiana, with immediate access for those struggling with intravenous substance use or pregnancy-related addictions. Community-based services extend support into homes, schools, and neighborhoods through skills training, wraparound programs for children with complex needs, school-based mental health supports for over 20 corporations, and addiction recovery including medication-assisted treatment and peer supports. Addressing social determinants of health, 4C Health offers residential programs like supervised group living and transitional housing, supported employment, permanent supportive housing at Sargent Place for families impacted by substance use and homelessness, and street outreach teams.
