4C Health, formerly known as Four County Comprehensive Mental Health Center since its establishment in 1975, serves as the designated Community Mental Health Center for Cass, Miami, Fulton, and Pulaski counties, with growth since 2015 to include Howard, Tipton, and White counties, becoming a Certified Community Behavioral Health Center for these seven rural Indiana counties in 2024.

Rebranded in 2022, 4C Health reflects its roots in four North Central Indiana counties while emphasizing whole person health through high quality, compassionate, collaborative, and competent care that supports all Hoosier well-being.
