4C Health, formerly known as Four County Comprehensive Mental Health Center since 1975, is a designated Community Mental Health Center serving the rural communities of Cass, Miami, Fulton, Pulaski, Howard, Tipton, and White counties in North Central Indiana, with a mission to support all Hoosier well-being through high-quality, whole health care that is compassionate, collaborative, and competent.
